HEALTH CLINICS OFA APPLICATIONS
OFA is now requiring all submissions to be electronic in order to receive the OFA clinic discount.
OFA Online is now available for electronic CAER applications with participating ophthalmologists as well as electronic submission of completed soft-tissue applications.
Go to https://online.ofa.org/login/. This will allow you to create an account.
Enter the registration number of each dog to see if they are in the system. If not enter the information for each dog. You can then print out the Patella and Basic Cardiac forms that will be automatically filled in with your dog’s information, and bring them with you to the clinic.
At the health clinic we will submit each form to OFA. You may take a photo of the completed forms for your records.

Eye exams should be done at LEAST once on EVERY Border Terrier, and yearly on all breeding dogs.

SLEM Testing:
SLEM test kits: We swab if you order the kit and pay ($65) on line at OFA -- https://ofa.org/order-dna-tests/ .
You must bring the kit with you to the clinic.
We will make sure the dogs are microchipped, scanned, and will assist with swabbing, and transfer to the FTA papers. FTA papers have to dry completely prior to shipment.
We will ship the completed SLEM test kits with the CHIC Blood samples. All result paperwork will be submitted by us to OFA. We suggest you take a photograph of your results.

You will be notified of your appointment time. You are responsible for getting your dog there in time. No Refunds for no shows
